2021 Mother's Day Church Service
Please read the following and complete the form below:
DATE & TIME: May 9, 2021 at 11:00am
*Please arrive by 10:40AM for temperature check
We have 40 spots available for RSVP to attend the in-person church service. There is a max number of 7 members per family as to allow multiple families to attend - thank you for understanding. You must have a confirmed RSVP to attend. Email or text confirmations will be sent to officially reserve your spot.
PLEASE NOTE:
If you have any COVID symptoms or signs of any illness, please stay home.
Masks must be worn at all times.
Please arrive at 10:40am for a temperature check.
Waivers (paper or online) must be signed to enter the church
Follow markers and remain 6ft apart from individuals at all time.
Please wash your hands and use the sanitizing stations provided.
Please limit physical contact like hugging, high fives or shaking hands.
Children are welcome but are expected to sit with their parents at all times. There will be no class provided for the children.
No food will be served after service.
IF YOU ARE LATE and not onsite by 11:15AM, your spot may be given to a member on the waitlist. Please inform us if you must cancel your spot so we can release it to the next person.
